This was gotten at the Mansfield Phish show in 2014. I believe they were at every venue that tour.
In order to get one, you had to fill out your name and address on the back and pledge to vote. They then mailed you the postcard, with the bottom part chopped off at the end of the tour.
I was able to get a full one simply by saying I was getting it for a friend and I would bring it back signed later.
